Working together for the right outcomes
Homebrook Woods works collaboratively with Local Authorities, schools and professionals to ensure that every placement is carefully considered, appropriately planned and centred around the individual child.
A collaborative approach
We recognise the importance of partnership working and value the knowledge and expertise that every professional brings. By sharing information, maintaining open communication and working together, we aim to ensure that every child receives the right support at the right time.
We welcome early discussions with professionals to explore whether Homebrook Woods may be an appropriate placement and are always happy to discuss individual circumstances before a referral is made.
Our aim is to build positive, transparent relationships with all agencies involved so that children experience a joined-up approach throughout their placement.

Making a referral or enquiry
We welcome enquiries at any stage. Whether you are exploring options informally, considering a referral, or ready to discuss a specific child, please get in touch. We aim to respond to all professional enquiries within two working days.
Where appropriate, we are happy to arrange an informal visit to Homebrook Woods for professionals before a placement decision is made. We can also provide information on our commissioning and fee arrangements on request.
Make a Referral
Homebrook Woods welcomes referrals from Local Authorities, schools, professionals and, where appropriate, parents or carers. Our admissions process is designed to ensure that every child is considered fairly, carefully and individually to determine whether Homebrook Woods can meet their needs safely and effectively.

Please ensure you have:
- A completed Homebrook Woods Admissions Referral Form
- The child's EHCP or draft EHCP (where applicable)
- Recent professional reports
- Relevant risk assessments (where applicable)

Submit Your Referral
